Notice of License Form

A DSP that would like to obtain the Blanket License must submit a Notice of License (NoL) to The MLC. Click here to gain access to the form.

Notice of Nonblanket Activity Form

A DSP that would like to forego the Blanket License and is a Significant Nonblanket Licensee must submit a Notice of Nonblanket Activity (NoNA) to The MLC. Click here to gain access to the form.

Service Configurations Form (Stand-alone)

Before reporting usages to The MLC, DSPs are required to complete this form, either as part of the Notice of License Form or using this stand-alone form. This form ensures that The MLC applies the correct compulsory licensing rates to the DSP's services. Click here.
